Lost/Damaged Diploma

For information on issuing a replacement diploma certificate, please visit KAA at Campus II Babarsari with the following documents:

1. A request letter addressed to the Rector of UAJY, including contact number (HP/WA)
2. Three 4x6 black-and-white photos in formal attire (suit and tie)
3. Photocopy of the diploma (if available)
4. Police report for a lost diploma (when applicable) [see note below]
5. The damaged original diploma (when the request is due to a damaged diploma).

Documents can be sent by post/courier to the address:

Note:

•         Before making a police report (number 4), please request a cover letter of 'Lost Diploma Report' by emailing kaa@uajy.ac.id. You may directly write the request in the email body while including your complete personal details and WhatsApp
•          Students can go to the Faculty Administration Office to obtain a diploma legalization service.
•          The Replacement Academic Transcript Certificate is provided by each faculty.
